This survey will help us understand the problems and factors that shape people’s health in Dunn County. When you take this survey, your responses will help decide the biggest community health issues that Health Dunn Right, our local health coalition, will work on 2028 through 2030.

This survey should take about 5 minutes. You should only take it if you are an adult who lives or works in Chippewa, Dunn, or Eau Claire County. If you live in one county and work in a different one, you can take the survey twice if you choose (once for where you live, and once for where you work).

The survey is voluntary – no one is required to take it. Your answers will be anonymous and confidential. General results will be combined with all other survey answers when sharing. We will share the results of this survey with anyone who is interested in improving our communities’ health.

This survey will be open through October 2026.
